Recommended Vaccinations
Flu Shot (Influenza Vaccine)
Pregnant women, adults 50 years or older, and those with a chronic health condition (asthma, chronic heart or lung disorders or impaired immune system) should get the vaccine.
Pneumococcal Vaccine
All people 65 years or older, and anyone with a chronic disease (heart or lung disease, no spleen or damaged spleen) should get the vaccine.
Shingles Vaccine
Adults 60 years or older should get the vaccine to help prevent shingles.
HPV Vaccine (Human Papillomavirus Vaccine)
Women 13 to 26 years old should get this vaccine to prevent against four types of HPV, which can cause cervical cancer and genital warts.

Dr. Vijay Koli is very personable , he cares about me as a patient, and makes sure I get the very best care. He’s very thoughtful and makes sure I understand everything he discusses with me. He never rushes me if I have questions!
My appointment was at 11:30 I got called at 12:30 - not the worst part - Dr Brown just comes in sits at his computer and that's it. I don't ask anything because when I do he acts disturbed and talks so fast and low I can't understand him. I am going to find another doctor because I don't feel that I am getting the care I deserve.
I was blown away by the professional confidence and targeted questions that dialed in on my key symptoms and moved me to a care plan and follow up perimeters. This was my first experience with a nurse practitioner and a female health provider. I was self conscious of discussing male related issues, as I image girls must feel with a male doctor, but the manner at which she walked through the explanation calmed my nervousness, and put me at ease. Great Job!
Dr Vijay Koli is the best doctor I have ever had. I drive from out of town to see him when needed. He is very thorough and is not rushed so he explains everything and listen to you as well. I almost feel like he is a part of our family!

I met Dr Puri for the first time this week and like her very much! The only issue I have is that I wear hearing aids and I had a bit of trouble understanding her with her mask on…maybe I

Liz is awesome. She NEVER makes you feel like you don?t have her undivided attention. She does not push you out the door to get to the next patient. She genuinely cares about you and your input on what you feel you need even tho you don?t have the medical expertise.
Mindy Smith, NP that I see as my PCP is always and I do mean always knowledgeable about what’s going on with me and care about any condition I may bring to her concerning my health. She is a blessing to LMC and I wouldn’t want anyone else taking care of me and my family.

Dr. Koli is amazing! I highly recommend him. He is friendly, courteous and truly listens to his patients. He does not try to rush through your appointment to get to the next patient.
Dr Koli is probably the best primary care physician that I have ever had. Very knowledgeable, talks to patient on level that they can understand, and always discusses options on treatment (including pros and cons). I would highly recommend him!
Jessica D'Ancona has a very nice and friendly disposition, she totally seems invested in your care. She listens and asks about your health and concerns. She also lets you know she is here at anytime even after the visit should I have any questions or concerns. I feel like I am being heard and that she is totally here for her patients. As soon as seven days before an appointment, you can complete eCheck-in through MyChart. You can update information and electronically sign documents before your appointment to make the check-in process faster.
Arrive 15 minutes before your appointment.
Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.
Let us know as soon as possible if you have to cancel.
Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.
Payment will be due at the time of your appointment.
Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 50% discount.

Prescription Refills
Call and leave a message with your name, date of birth, prescription and strength, along with your doctor’s name and your pharmacy’s name and number. Allow 24 to 48 hours for us to process the refill. We won't contact you about the request unless there is a problem.
We cannot refill some prescriptions.
You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail-order pharmacies—you must request these refills.
Test Results
You can receive timely notifications and view lab results from tests you’ve had done through MyChart. Test results should be available within 7 to 10 business days.
Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with your appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.
Forms
Due to the volume of forms we process, please allow up to 10 business days for us to complete them. There may also be a charge for completing your form, or your provider may require that you come in for an office visit for assistance in completing your form.

You can exchange messages with your providers and read doctor’s notes from recent visits in MyChart. Keep messages short and to the point. We will get back to you as soon as possible.
