I always feel like my voice and concerns are not being heard or over looked. She gets snappy at me when I try to explain my symptoms and always rushed saying we are running out of time after only 10 minutes into the visit only being able to adjust medication and not talk about any of my other problems or concerns. Last appointment she snapped at me because I asked a question when she was inputting information instead of asking for a moment to input the medication adjustment.

Dr. Pham has always taken care of my many health issues each and every visit I've had with her!!! She was supposed to be a interm provider outside of the VA but she has now become my main primary care provider even though I'm paying out of pocket for my appointments with her. I definitely recommend her for any and all who wants a provider who truly listens and cares and will address all problems you bring to her! Paula Allen explained my test results and how they had greatly improved. She discussed that my water intake had decreased and needed to increase. I asked about my leg veins that had been noticed by my massage therapist and referred me to check on them. Paula?s sincere and thorough assessment is reliable and accurate. I referred another person to this office and provider. I am very grateful for the entire staff.

As soon as seven days before an appointment, you can complete eCheck-in through MyChart. You can update information and electronically sign documents before your appointment to make the check-in process faster.
Arrive 15 minutes before your appointment.
Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.
Let us know as soon as possible if you have to cancel.
Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.
Payment will be due at the time of your appointment.
Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 50% discount.

Prescription Refills
Call and leave a message with your name, date of birth, prescription and strength, along with your doctor’s name and your pharmacy’s name and number. Allow one to two business days for us to process the refill. We won't contact you about the request unless there is a problem.
We cannot refill some prescriptions.
You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail-order pharmacies—you must request these refills.
Lab/Test Results
You can receive timely notifications and view lab results from tests you’ve had done through MyChart. Test results should be available within 7 to 10 business days.
Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with your appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.
Forms
Due to the volume of forms we process, please allow up to 10 business days for us to complete them. There may also be a charge for completing your form, or your provider may require that you come in for an office visit for assistance in completing your form.
Immunization Records
Please allow up to two business days on all immunization record requests. If you must call our office for an immunization record, you may be asked to leave a message with the patient’s name, birth date, and contact phone number.
School/Work Excuses
School/work excuses will be issued at the provider’s discretion only when the patient has had an appointment.
Questions for Your Care Team
You can exchange messages with your providers and read doctor’s notes from recent visits in MyChart. Keep messages short and to the point. We will get back to you as soon as possible.
After hours there will be an on-call provider who will be available for urgent needs that cannot wait until the next business day. If an emergency occurs after hours, please have paper, pencil and pharmacy phone number available. Prescription refills will not be handled by the on-call provider nor will pain medications, controlled substances or antibiotics be prescribed to your pharmacy.
