Your Responsibilities
Know the Details of Your Plan
It’s your responsibility to know your co-payments, deductibles, referral guidelines and what services are covered.
Secure Prior Approval
If your insurance requires pre-certification, pre-authorization, or other approvals, it is your responsibility to secure them.
Pay Non-Covered Charges
You are responsible for paying all charges not covered by insurance, including deductibles, co-pays, per-stay co-pays, and other non-covered charges.
Pay Unpaid Charges
You may be required to pay for any unpaid health care items or services that your insurance plan deems non-covered or not medically necessary.

Filing a Claim
We will file a claim for your care with your insurance carrier as long as we have complete billing information and benefits are made payable to Lexington Health.
We will only contact you about your claim if you owe a balance or we need your help obtaining payment from the insurance company.
